REGISTRATION AND FORMS
The Tracy Unified School District utilizes the web based portal Family ID for student athletes and their families to register in each sport. Once you login through this link you will be able to access the medical and insurance forms required for participation. The portal also serves as the mechanism in which to pay the registration fees directly to the school.

Note: Regarding the medical clearance, a new process is in place. A clearance from the doctor on their letterhead needs to be provided.
Special Use Forms (only required if driving/assisting)
Once filled out, these special forms will be provided to the Principal with a printout of the DMV record, proof of liability insurance, registration, and also verification of a TB Test.
The HR department will contact applicants to arrange the Department of Justice fingerprinting and background check.
